GCMH Offers Sports Physicals on August 2nd, 2010

On Monday, August 2nd from 5:00 p.m. – 7:30 p.m., sports physicals will be offered to area high school students at the Grundy County Memorial Hospital (GCMH). The cost is $30.00 and is due at the time of the physical. The event is a fundraiser to support the Grundy Center High School (GCHS) Athletic Trainer program. Students must bring their Pre-participation Physical Examination Form, along with the Sports Physical Permission Form, to the hospital with them. The health history section of the physical form should be filled out prior to the sports physical.
Participants will receive a comprehensive physical including: medical evaluation, physical therapy and chiropractic screening, vision screening, and exercise prescription (if needed).
